- Izual: Dropping quality increased. Has a chance to drop ‘Dimensional Blade’ since Nightmare Difficulty, has a chance to drop ‘Phase Blade’, ‘Dimensional Shard’ and ‘Hell Warden’s Husk’ in Hell difficulty. Activated an abandoned taunting soundtrack.
- The Tormentor: (fire enchanted, magic resistant) A Will-o-Wisp champion locates beside Izual, Lightning damage almost same as vanilla, can open wounds in Nightmare and Hell difficulties. Has a chance to drop ‘Wisp Projector’ in Hell difficulty.
- Riftwraith the Cannibal: (teleportation, cold enchanted) Base stat and dropping quality increased.
- Hephasto the Armorer: (aura enchanted, spectral hit) Collide size increased to 3 to make him easier to kill with ‘Charged Strike’, melee with ‘Bash’, has a chance to drop ‘Skin of the Flayed One’ in Nightmare and Hell difficulties.
- World Killer: (aura enchanted, teleportation) A Blood Maggot Queen dwells on the bridge to the Chaos Sanctuary. She is more powerful than other Maggot Queens as she can launch a tentacle attack and sprays poison gas around like Mephisto.
- Darkwing: (extra strong, cursed) A Balrog champion guarding the entrance of Chaos Sanctuary with a pack of Guest Pit Lord minions, can additionally melee with ‘Frenzy’ comparing to common Balrogs.
- Grand Vizier of Chaos: (fire enchanted, extra strong) Base stat and dropping quality increased, no longer flying so it won’t fly out of the stage and break the mission.
- Lord De Seis: (aura enchanted, extra strong) Can summon more ‘Doom Kights’ instead of casting ‘Weaken’. Has a chance to drop ‘Crown of Thieves’ since Nightmare difficulty.
- Infester of Souls: (extra fast, spectral hit) Can additionally melee with a ‘Rabies’ Logic poison skill comparing to common Balrog.
